MEDICAL ASSISTANCE

Eligibility

Assistance from Chief Minister’s Relief Fund is given to :

• Permanent residents of the State of West Bengal.

• Distressed patients who are undergone treatment in the Govt./Non Govt. Hospitals/Institutions and also are unable to bear the cost of treatment

• Patients who are not eligible to get benefit of Employees State Insurance Scheme/any other insurance scheme or get reimbursement  from employer/organization.

• Patients whose annual family income does not exceed Rs. 1,20,000/-

• Patients can avail the assistance only once for the same case in one financial year.

 How to Apply

People who seek financial assistance from Chief Minister’s Relief Fund may apply online through https://cmrf.wb.gov.in or may send their signed (in case of LTI it should be attested by Govt. Group-A Officer) application on  plain paper along with all related documents to :

Application

Following are the required documents for financial assistance from Chief Minister’s Relief Fund for medical treatment.

• Prayer addressed to the Hon’ble Chief Minister for financial assistance duly signed by the patient / patient’s guardian. Complete address, mobile number and e-mail ID (if any) might be mentioned in application.

 • Recommendation to the Hon’ble Chief Minister from local MLA / MP.

 • Family Income Certificate issued by competent authority (Minister-in-Charge / Minister of State / DM / SDO / BDO / Joint BDO / Executive Officer in case of Municipality Area / Officer in the rank of Deputy Commissioner and above in case of Municipal Corporation area).

 • Estimate (for present & upcoming treatment up to one year) of current date from Hospital/Nursing Home indicating tentative O.T. date in case of operation, if any. This estimate must be signed by the treating Doctor/Department Head along with stamp/seal and countersigned by the Superintendent/Director/Manager/Proprietor of the respective Hospital/Nursing Home along with stamp/seal .

 • In case of kidney transplantation, affidavit from 1st Class Judicial Magistrate for Blood Related Donor or No Objection Certificate (NOC) from Health & Family Welfare Department, Govt. of West Bengal for Non-Blood Related Donor.

 

Sanction & Disbursement

Applications for sanction of financial assistance from Chief Minister's Relief Fund are processed and examined by the CMRF Cell of Chief Minister's Office. The Application may be sent to District Magistrate or Commissioner of Police, Kolkata, as the case may be, for enquiry. Hon'ble Chief Minister, West Bengal sanctions financial assistance from the fund to partially defray the expenses of treatment. The financial assistance is sanctioned once in a year. Generally the sanctioned amount is directly forwarded to the concerned hospital / institute as per their advice. The sanction of financial assistance is provisional, Authority has sole discretionary power to cancel the prior sanction without assigning the reason thereon.

EDUCATIONAL ASSISTANCE

Eligibility

Assistance from Chief Minister’s Relief Fund and Chief Minister’s Discretionary Fund is given to the student :

• Who is a permanent resident of the State of West Bengal.

•Who is studying in any institutions of the State after passing out the State Board / Council of Secondary / Higher Secondary Education or a State aided University in West Bengal.

•Who has got 50% and above but less than 60% marks in aggregate in Madhyamik Examination [for H.S. level] or 50% and above but less than 60% in aggregate in H.S. Examination [ for U.G. level] or 50% and above but less than 53% marks in Graduation [for P.G. level]

•Who do not enjoy any other governmental (central/state) scholarship or stipend for the same course or stage of study during the last year.

• Whose annual family income does not exceed Rs. 1,20,000/-.

Following are the required documents for financial assistance from Chief Minister’s Relief Fund for financial assistance.

1. Prayer addressed to the Hon’ble Chief Minister for financial assistance duly signed by the applicant. Complete address, mobile number and e-mail ID (if any) should be mentioned in the prayer.

 2. Recommendation to the Hon’ble Chief Minister from local MLA / MP

 3. Family Income Certificate issued by competent authority (Minister-in-Charge / Minister of State / DM / SDO / BDO / Joint BDO/Executive Officer in case of Municipality Area/Officer in the rank of Deputy Commissioner and above in case of Municipal Corporation area)

 4. Self-Declaration by the student under his/her own signature stating the present course of study along with year/semester and receipt of any Govt. / Non-Govt. Scholarship / Stipend. This declaration should be countersigned by the head of the present institution with stamp / seal

 5. Admit Card of 10th standard examination.

6.Mark Sheet of qualifying examination (for assistance in H.S. Level the qualifying examination is Madhyamik or equivalent; for assistance in U.G. Level the qualifying examination is H.S. or equivalent and for assistance in P.G. Level the qualifying examination is Graduation).

 7. Rank card of the entrance examination for professional courses like Diploma Eng. / B.E. / B.Tech. / MBBS / Law / Nursing / Pharmacy.

 8. Allotment letter for admission in above said professional courses.

 9. First page of Pass Book of applicant’s active savings Bank Account where A/c. No., IFSC No. and Account Holder’s Name is clearly visible.

 10. Admission receipt for current academic year / semester.

 Sanction & Disbursement

Request for sanction of financial assistance from Chief Minister’s Relief Fund is processed and examined by the CMRF Cell of Chief Minister’s Office. The application may be sent to District Magistrate or Commissioner of Police, Kolkata, as the case may be, for enquiry. Hon’ble Chief Minister, West Bengal sanctions financial assistance from the fund. The financial assistance sanctioned is a one-time grant only. The financial assistance is directly credited to their Bank account. The sanction of financial assistance is provisional, authority has sole discretionary power to cancel the prior sanction without assigning the reason thereon.

CM’S RELIEF FUND

 General Details

 

Objectives

The Chief Minister’s Relief measures to the distressed. Hon’ble Chief Minister of West Bengal sanctions financial assistance to the poor who are in distress due to reasons such as:

• Health problems which require expensive Medicare.

• Financial scarcity of meritorious students for pursuing higher education.

• Loss of properties and sources of livelihood due to unforeseen incidents, natural calamities, road & fire accidents etc.

 

CMRF Cell

Chief Minister’s Relief Fund Cell, a specialized Cell in the Chief Minister’s Office

• Process all the application and disburse the sanctioned money.

• Maintain proper accounts for the receipts and expenditures, which is audited every year.

Donation

The Chief Minister’s Relief Fund has been built up by donation / contribution made by humanitarian and socially committed persons or organizations. The donation or contribution made to this fund is entitled for 100 % exemption from Income Tax under Section 80G(5)(VI) of I.T. Act,1961.

All donations, big or small, are most welcome

The donation may kindly be sent either by Cheque or Demand Draft drawn in favour of  CHIEF MINISTER’S RELIEF FUND.

Specific cause for donation may be mentioned in the forwarding letter to enable utilization accordingly.

Receipts will be given only after the credit of the amount to the Chief Minister's Relief Fund.
